3 steps to submit your claim

Get your documents ready

Make sure you have all the documents and forms needed before you submit your claim.

Download and complete the forms:

Insurance Claim FormInsurance Police Report Form

Documents needed:

  • Certified ID copy of the deceased
  • Certified ID copy of the nominated beneficiary
  • Certified computerised death certificate
  • Notice of death (BI1663)
  • Claimant's bank stamped statement/letter (not older than 3 months)
  • Letter of authority (if no beneficiary is listed)

Send us your documents

Submit your claim in one of these ways:

We'll send you an SMS within 24 hours to let you know we received your claim.

We'll process your claim

  • Once you have submitted all the correct documents, we'll process your claim within 48 hours.
  • If anything is missing, we'll send you an SMS to tell you what we still need.
  • You have up to 6 months to submit a claim from date of death.
